This page describes how Art of Animals Chiropractic handles information someone sends through this website. Questions can go to doc@artofanimalschiro.com.
What the forms collect
The site collects information when a person submits one of these forms.
The new patient form asks for the owner’s name, street address, city, state or province, postal code, phone number, and email; the primary veterinarian’s name and phone number; and the animal’s name, species, breed, weight, age, and birth date. It also asks why a visit is being requested and for history the owner chooses to share: behavior, whether a veterinarian has given a diagnosis and what it was, when and how an issue began, eating and drinking, urine and bowel movements, past injury, cancer questions, unexplained weight loss, prior chiropractic or laser care, whether the problem has changed, medications and supplements, surgeries, how long the animal has been in the home, how the animal joined the family, other animals in the home, and how the owner heard about the practice. It asks whether Art of Animals Chiropractic may use photos or videos of the animal in its own materials, and it asks for a typed signature. Photos or video files are optional.
The vet referral form asks for the referring veterinarian’s name, clinic name, clinic phone number, and clinic email; the animal owner’s name, phone number, and email; the animal’s name, species, and age; the reason for the referral; whether the clinic wants the initial visit notes by email; and a consent checkbox for examination and treatment of the animal’s musculoskeletal system. Photos or video files are optional.
The contact form asks for a first name, last name, email, and message.
How that information is used
Art of Animals Chiropractic uses a submission to reply to a message, prepare for a requested visit, or follow a veterinary referral. A yes on the photo and video question is used only for the practice’s own materials. A no is not treated as permission.
The notice at the bottom of the page
This site does not run an analytics or advertising script, and it does not set a tracking cookie. A short notice sits at the bottom of the page. Closing it saves one value in this browser’s local storage so the notice stays closed on later visits. That value stays on the device. It is not sent to the practice. Clearing this site’s stored data in the browser brings the notice back.
Who else receives it
Form submissions are sent through Netlify, the service that hosts this site, so the practice can read them. This site does not pass form answers to an advertising service, and it does not sell them.
Pages load typefaces from Google Fonts and photographs from Squarespace’s image service. The browser contacts those services to display the page.
